Home From Hospital

Home from Hospital is a FREE service helping patients make a seamless transition from hospital to home by providing a variety of help and support, as well as giving reassurance and building confidence.  

The service is available for patients living in North Yorkshire or the East Riding of Yorkshire.

North Yorkshire  - covering Scarborough, Whitby, Ryedale, Hambleton and Richmondshire - refer here (Delivered by Carers Plus Yorkshire)

Who is Home from Hospital for?

Who is Home from Hospital for?

Adults aged 18 and over that:

  • Have had an admission to hospital
  • Have had a day case procedure
  • Have been seen in the A&E department
What the service provides

  • Eessential shopping
  • Prescription collection
  • Telephone/home visit support
  • Liaising with Health and Social Care professionals
  • Helping to set up services and support for the future
  • Providing emotional support for family and friends that are caring for a person that has been recently discharged from hospital
What the service cannot provide

What the service cannot provide

  • Personal Care (including washing, dressing and toileting)
  • Medical Care (giving medication and other medical care)
  • Respite Care (including complex needs)
