Home from Hospital Co-Ordinator

I am part of the Home from Hospital team based in the Hambleton and Richmondshire area. My role is to support those aged over 18 that have recently been discharged from hospital. I like meeting new people and making a difference to their lives. My role is varied and rewarding; I can honestly say that I love my job!
